How to Open a URL (with #) upon Incoming Call?

GOAL: Open
http://swiftcloud.me/addressbook/address?phone=1234567890

for all incoming phone calls.
This will bring up that client's record, or instantly create a new record if none exist for that phone, or show a list if multiple contacts have the same phone ID (i.e. a business).

Possible? How?

I'm using Google Voice if that matters.
