That was not applicable to me. (My router doesn't have SIP-ALG)

I did that, but it didn't seem to change anything.  And besides, I don't think the ports are actually open because when I use an "Open Port Check Tool", it always says that the ports are closed.

So I asked them:
"If the port forwarding worked, wouldn't the response be that the ports are open?"
But they didn't answer me about that.

I then decided to replace my router with a newer version and the *78/*79 started to work! :)  But I did ask them:
"When we do dial *78, it is immediately followed by a fast busy signal ... beep beep beep.  And that beep beep beep is ALWAYS on the line until we dial *79; is that normal?"

And they said:

I did ask them if there was a way to make it so that there isn't a constant beep beep beep, but they didn't answer.

So, so far, the star codes I've tried are working!  It must've been a router issue then.

Whew; what a long, arduous route just to learn that new equipment would be the answer.
