misroutes calls

I have a 508 and when I transfer a call it transfers to all extensions rather than the extension I have selected.

If I  go #n and n = 14 the call goes to 11.
If I  go #n and n = 12 the call goes to 11.

The same is true for
If I  go ##n and n = 14 the call goes to 11.
If I  go ##n and n = 12 the call goes to 11.
