PhonePower SIP Setup Fails with "Register Failed: 400 Invalid From" message

I've been using a Obi200 for years with PhonePower without any problems.  Last night lightning fried my Obi200.  Luckily I had a spare so I added it as a new device.  However, the problem comes when I trying to setup my SIP account.  I'm getting a "400 Invalid From" error for some reason.  Can anyone please help?  Thanks!

Are you sure you are entering the correct username and password? Usually they are not the same as those to log in to the account via the ITSP's web site.

If it isn't the same where would I find the correct credentials?

I'm not a PhonePower customer so don't know the specifics but typically that info can be found by looking at your account on the ITSP's web site or they may be in an email sent to you by the ITSP when you originally signed up with them.
