Setting up Cisco 8961 IP Phone with OBi200

I have a Cisco 8961 IP phone that I want to set up with my OBi200.

I have try some settings that were suggested from this topic:

but for this phone I am not able to get pass the "Phone Not Registered", and I am not sure how to configure the phone to receive and dial out calls.
