Placing calls on Broadvoice (RESOLVED)

I tried setting up Broadvoice on an Obi100 using the Broadvoice option on OBiTALK but it didn't work for me, the device never registered. I got the device to register using a generic service provider on SP1 and so far I can receive calls fine, the problem is making calls. The SP1 is set as the default and I've tried prefixing the number with **1 and direct dialing but in every case I get a male voice saying "the number you dial **** was rejected by the service provider, reason is 403". The call log is below (phone number changed to @ to protect privacy). Can someone help?

Log into the OBi directly using the IP address returned by dialing ***1 (username/password = admin/admin unless the password was changed by the OBiTALK Web Portal) and verify the following settings (everything else should be at Default):
Service Providers -> ITSP Profile A -> General -> Name : (BroadVoice)
Service Providers -> ITSP Profile A -> SIP -> ProxyServer : proxy.$$$.broadvoice.com
Service Providers -> ITSP Profile A -> SIP -> OutboundProxy : proxy.$$$.broadvoice.com
Voice Services -> SP1 Service -> AuthUserName : (10 digit BroadVoice number)
Voice Services -> SP1 Service -> AuthPassword : (10 digit authentication Password generated by BroadVoice)
Voice Services -> SP1 Service -> CallerIDName : (callerid name)
Physical Interfaces -> PHONE Port -> PrimaryLine : SP1 Service
where $$$ is one of the following:
lax
dca
mia
atl
chi
bos
nyc
You should not need to use **1 when dialing numbers for SP1.
If you find it necessary to change anything and wish to make that change in the OBi instead of on the OBiTALK Web Portal, you will also need to set:
System Management -> Auto Provisioning -> ITSP Provisioning -> Method : Disabled
System Management -> Auto Provisioning -> OBiTALK Provisioning -> Method : Disabled

Ron, I appreciate the help. I verified that all settings (save for the profile name) are as you suggested yet I still can't place calls. I'm using lax as the server which is what I had with the old Sipura ATA. The device registers and receives calls so the username/password should be correct. Cound there be another setting affecting outbound calls?

If possible, temporarily bypass your router by connecting the OBi directly to the modem. Power-cycle both the modem and the OBi after making the direct connection, allowing the modem to acquire sync before powering up the OBi. Then see if you can make outgoing calls.

Please post a screenshot of all Line page settings in the Sipura. Of course, mask personal info such as phone number and name (password should already be masked), but leave all other settings visible.
