I have recently applied the latest firmware and I believe it to be the cause of a couple of issues:
1) Whenever I have to dial a party which requires me to introduce an extension, the extension number is not received on the other side properly. Most of the time I keep getting invalid number. This is also the case when I dial into an audio conference. The number to get onto the bridge works fine, but once on the bridge I need to enter the conference code, and most of the time I'm getting an error. This is quite a critical issue for me at the moment.
2) I'm not sure if this is prompted by the new firmware, but I think it might be, as I believe it worked before: Call forwarding (*72) doesn't even blink. I use the OBI with Google voice, and I need to set-up the forwarding on on my side (either on no answer, or just always foward). It just doesn't do that.
