Help Troubleshooting OBI100 for Incoming Calls

Sorry for the newbie question, but I don't know how to troubleshoot this.
I can make calls but there's no voice transmission. And I can't dial into the auto attendant from my OBiTalk app.
It used to work fine.
I didn't use it for quite a while, but now I need it again. It dials in OK from OBiON, but no auto attendant - it rings through to the phone jack. When I pick up the call (using an extension on my PBX or a POT plugged into the OBi phone jack) it answers the call but there's no voice either way.
If I don't pick up the call it goes to my Google Voice voicemail.
I did a factory reset. Same behavior.
Any suggestions? Has the OBi gone bad?
