I followed these instructions, (installed the Hangouts extension in Chrome).  When I click on the chat avatar in Gmail, I don't see any option for making a call.  Should I revert to old chat? 

Revert to old chat and it should work.

Tried making calls from Gmail multiple ways numerous times, still no chat option.

Note to Obi, these are things that customers need to know up front, existing number required, dedicated Google Voice account, problems with Google chat, etc.

Obihai makes fairly low priced VOIP hardware for use with many different providers, asking them to provide "detailed" instructions for every supported service seems a bit much for the price imo.
