This setup work about 50 percent of the time but I can not get it any better. Need some help.
VOIP Setup – for Latitude Pacemaker Communicator

Add a star code *99 as this:
*99, Modem Call, set($Noji1,200),set($Noec1,1),set($Cdm1,1),call(18002975493)
The 3 set() commands tell the obi to do the following for the next outbound call:
1. Disable Jitter Buffer Adjustment, and use a fixed jitter buffer length of 200 ms
2. Disable Echo Canceller
3. Use only G711u codec
Change Codecs
G711U PacketizationPeriod = 10
G711A = Uncheck Enabled
G729 - Uncheck Enabled
G7296R32 - Uncheck Enabled
All other settings are OBI200 Default
Reboot ATA Unit
Boston Scientific Latitude unit
Turn on switches 2 and 3 which for the unit to send the *99 code

This may be too late since this is an old topic but here goes ..
I had a similar situation with the same cable modem but a different Latitude Communicator. My Latitude box was the old one that had no switches and just the phone jacks. I tried for weeks to get my device to communicate reliably through a VOIP box and provider, but I never did find anything I could say was a 100% fix. I was NOT using an OBI by the way.
Anyway, I thought I was going to be locked into my stupid land line phone charges forever until I found on the Boston Scientific website that there is a new Latitude Communicator that connects to your router via a USB ethernet adapter. The new Latitude Communicator box was free, but the ethernet adapter costs $75. Which is nothing considering I was paying almost $30 a month + taxes for my phone line. I did not need doctors approval and it was all handled in one phone call which lasted maybe 10 minutes.
The new box arrived by FedEx in two days and I was up and running in under 5 minutes. I submitted the $75 charge to my insurance, but I haven't heard back from them yet.
Hopefully you'll see this and maybe get one of the new Latitude boxes to help your situation.
