Can't transfer call to voicemail

Our office has been using the Obi1000 for the past several months and not without problems. The most frustrating is the seeming inability to transfer an incoming call to someone's voicemail.

As far as I know, I'm transferring the calls properly.

1. The call comes in.
2. I dial the other extension (without putting the incoming call on hold) and press the "transfer" key.
3. The call is lost.

If the other party is in the office, there is no problem connecting. Others have also tried the "blind transfer" option with the same negative results. Can anyone suggest what I'm doing wrong (actually, it's not just me) or what setting needs to be corrected? Incidentally, I do receive voicemails via email but I have no idea how those calls are reaching me. Thanks.
