Google Voice dreaded "...has not received a response from the provider" message.
Same number worked from other device to same number.
Still failed after the other number worked.
OBiTalk showed normal status and Configured.
Other Google SPx on this same 202 worked to same phone number.
I deleted the SPx slot then reprovisioned the same GV number to the same slot.
It took about 3X time to come up as Configured.
Note: An interim page refresh on OBiTalk showed "Registration Not Required" for ALL SPx positions (all 4 in use with GV)
Total time about 15 mins to troubleshoot and reprovision.
I suspect this is a service disruption in one or more services.
As a rule of thumb, never try to mess with the OBiTALK portal on weekends. It is often broken until somebody comes in and deals with it during business hours.
