Title: OBi100 phone port status vbat 0v, tipringvoltage 0v

I'm stumped on this setup for a 100 after setting up a 202 in a couple minutes without issues (on same router and phone combination, i.e., no firewall changes, etc., and phone is functional).  I'm thinking it's a hardware issue.  Below are a few points regarding the setup and a few things I've attempted:

Device: OBI100
Phone icon on device: solid green
Firmware: 1.3.0 (Build: 2744)
SP1 service status: registered, provider admin page also shows registered
Phone Port Status:
- state: on hook
- vbat: 0 v
- TipRingVoltage: 0 v

- no dial tone
- no result dialing **9 222 222 222

- updated the firmware, no change in the result
- tried increasing the TipRingVoltage (after reading some posts) to 50 v (max) with no result; reverted to default
- tried switching the Primary Line between SP1, SP2, and Trunk Group 1, no change in result
