Some Obi calls go right to Google Voicemail

I use GV, which forwards to CC and then to my Obi.

I just noticed a call that never made it to my Obi (no phones rang) but Google Voice did take a message.

I'm not even sure where to begin to track this issue down.

You can't find out anything at google voice. You can check your call history at CC just for the heck of it. It is possible all sessions were busy on your Obi or on CC when the call came in.

In the years I have used it, I have found google voice to be 99.9% reliable for me in doing exactly what it is supposed to do.
